The development of PM in CRC results in an increased rate of deaths among CRC patients. A different form of metastasis called PM affects the membrane that is enclosed in the abdominal cavity, known as the peritoneum. As a result, the accumulation of fluid may arise in the abdominal area, leading to ascites and other complications, including pain and bowel obstruction. Studies show that anywhere between 10% and 40% of CRC patients would subsequently develop PM with a life expectancy below a single year once PM is diagnosed.   Several factors increase the risk of PM, including:
  • Tumor Stage: Patients with stage III or IV CRC face a higher risk of PM compared to those with earlier-stage disease.

  • Tumor Type: Mucinous and signet-ring cell carcinomas have a higher tendency to spread to the peritoneum than other CRC types.

  • Lymphovascular Invasion (LVI): The presence of cancer cells in blood or lymph vessels (LVI) elevates the risk of PM.

  • Peritoneal Involvement: A high risk of PM development exists in patients with cancer cells on the peritoneum at diagnosis.
To mitigate the risk of PM, consider these strategies:
  • Early Diagnosis and Treatment: Diagnosing and treating CRC promptly can reduce the risk of PM. This involves removing the tumor and any affected lymph nodes as soon as possible.

  • Hyperthermic Intraperitoneal Chemotherapy (HIPEC): HIPEC, a chemotherapy delivered directly to the peritoneum, often complements surgery in treating PM.

  • Ayurvedic Therapy: Ayurvedic management of colorectal cancer, especially in reducing PM risk, involves a holistic approach that includes diet, lifestyle, and herbal remedies.

  • Targeted Therapy: This treatment targets specific cancer cell molecules and can be effective against PM.

  • Immunotherapy: Immune therapy mobilizes the immune power to fight cancer with possible effectiveness for the treatment of PM.
Lifestyle changes also play a crucial role in reducing cancer risk:
  • Healthy Diet: Consuming less red meat, fewer processed foods, and sugars and more fresh vegetables, whole grains and perhaps fruits will cut the chances of cancer.

  • Healthy Weight Maintenance: Weight control cuts back on cancer.

  • Regular Exercise: Frequent physical activity lowers cancer risk.

  • Adequate Sleep: Sufficient sleep is important for cancer risk reduction.
In conclusion, mitigating the risk of peritoneal metastases in colorectal cancer demands a comprehensive approach that integrates effective medical treatments, precise surgery, lifestyle modifications, and possibly complementary therapies like Ayurveda. Collaborating closely with healthcare professionals to create a personalized treatment plan is crucial.
